Revive Health
Virtual Primary Care
You have 12 virtual primary care visits per member per year, as well as 12 care visits per covered child per year!
Virtual Urgent Care
You have 12 virtual urgent care visits per member per year.
Prescription + Pharmacy Care
Over 1,000+ generic medications are included and available with your membership. A list of these medications can be found by accessing the Revive Health member portal. Prescriptions can be filled at a local pharmacy or direct mailed.
Virtual Mental Health Therapy
You have 12 mental health therapy visits per member per year. Mental health therapy available for adults and covered adolescents ages 12 and over.
Additional Health Support
If you have challenges or need assistance and support accessing things like food, shelter, transportation, childcare, job training, etc. you can call our dedicated Customer Care team as a Revive Health member and get available resources to support your needs.
Lab Testing
If your doctor orders lab work, orders will be sent to the nearest Quest or LabCorb facility where you can have the lab work completed at a discount up to 80%. Labs qualify for HSA or FSA payments.

Additionally, depending on your employer's policies, you may have to wait until the next open enrollment period to join, which could mean going without certain benefits in the meantime. What is Revive Health?
Revive Health is a virtual medical subscription service that provides employees with access to a wide range of healthcare services. These services include virtual urgent care, primary care, mental health and wellness support, workplace injury triage, and various educational tools.
Can I participate if I already have insurance?
Yes, you can absolutely participate even if you already have insurance. You'll gain additional benefits at no net cost to you!
Is it possible to opt-out of these benefits?
Yes, you can waive these benefits, but please note that if you do, you’ll have to wait until the next year's open enrollment period to sign up again. If you’d like to waive coverage or have any questions, feel free to give us a call—we're here to help!
